TD Meloche Monnex Consent Form
Important information for clients with existing home or auto policies under our old program with TD Meloche Monnex.
In November of 2010, you should have received a letter from TD Meloche Monnex informing you of our decision to move our Home and Auto insurance program to The Personal. Included with this letter was a consent form that could be used to give TD Meloche Monnex permission to share your existing policy information with The Personal so that they may contact you to arrange a quote under our new program.
If you’ve already signed and returned your consent to TD Meloche Monnex, there is no further action required on your part. Please expect to be contacted by The Personal within 30 days of your 2011 policy expiration date to arrange for a competitive quote.

Providing your consent helps ensure that you have an opportunity to compare rates and benefits before the expiration of your current policy.
